Federal medical library
The United States National Library of Medicine (NLM) is the U.S. federal medical library and a component of the National Institutes of Health. Based in Bethesda, Maryland, it preserves biomedical literature and develops widely used information systems, including PubMed, MEDLINE, and PubMed Central.1
The National Library of Medicine is both a research library and a national infrastructure agency for biomedical information. Its institutional predecessor, the Library of the Surgeon General's Office, was established in 1836 within the U.S. Army Surgeon General's Office. The collection expanded through exchanges, acquisitions, and contributions from physicians and scientific societies.
Congress transferred the library to the Public Health Service in 1952, and the National Library of Medicine was formally created in 1956. It became part of the National Institutes of Health in 1968, linking its library, information-science, and research functions to the broader federal biomedical research enterprise.1
The modern NLM campus in Bethesda includes reading, archival, exhibition, and research facilities. Its historical collections document medicine, public health, nursing, and the social dimensions of health care, rather than serving only as a repository for current journal articles.
NLM makes biomedical research discoverable through interconnected databases, vocabularies, and full-text repositories. PubMed provides free searching across biomedical citations and abstracts, while MEDLINE is its largest component and uses the controlled vocabulary Medical Subject Headings, or MeSH, to improve retrieval across changing terminology.24
PubMed Central is a digital archive of freely available full-text biomedical and life-sciences journal articles.3 NLM also supports books, historical manuscripts, images, audiovisual materials, and data-oriented resources. These services serve clinicians, researchers, students, librarians, journalists, and the public, but a database record or retrieved article is not itself a guarantee that a study is clinically sound.
Indexing is a distinctive part of NLM's work: trained specialists and automated tools organize literature so that users can search concepts such as diseases, anatomy, therapies, and study types even when authors use different words.
NLM operates national-scale information infrastructure through the National Center for Biotechnology Information and other specialized programs. NCBI hosts PubMed, PubMed Central, genomic and molecular databases, and programming interfaces that allow researchers to retrieve and reuse records at scale.12
The library also provides authoritative information for particular audiences and emergencies. Its Disaster Information Management Research Center gathers resources for disaster medicine and public-health response, while toxicology and environmental-health programs have supported access to chemical, occupational, and exposure information. The National Network of Libraries of Medicine extends training, outreach, and resource-sharing through regional and national partnerships, helping health professionals and communities use reliable health information.
Many NLM services are openly accessible on the web, but their audiences and evidentiary roles differ: bibliographic indexing, archival preservation, research data, and consumer health education are related functions rather than interchangeable ones.
NLM's lesser-known work includes preserving rare medical books, manuscripts, prints, photographs, and ephemera alongside its digital databases. The History of Medicine collections include materials that illuminate medical practice, scientific institutions, war, epidemics, nursing, and the experiences of patients and marginalized groups.1
Its information systems also depend on standards that are easy to overlook. MeSH is revised annually, allowing new diseases, technologies, and concepts to enter a structured terminology system; this makes it useful for systematic searching but means that search strategies may need updating over time.4
NLM's digital reach does not eliminate access barriers. Articles may be indexed without being freely available, older materials may require specialized searching, and biomedical terminology can make public use difficult. Librarian training, outreach networks, and historical interpretation therefore remain part of the institution's mission, not peripheral services.
