Simvastatin

Simvastatin is a statin medication used to lower cholesterol and prevent cardiovascular disease. It belongs to the class of HMG-CoA reductase inhibitors, which block the enzyme that catalyzes the rate-limiting step in hepatic cholesterol synthesis. Simvastatin is a prodrug, administered as an inactive lactone that is hydrolyzed in the liver to its active β-hydroxy acid form. It is widely prescribed for primary and secondary prevention of myocardial infarction, stroke, and other atherosclerotic events, and is available as a generic under brand names such as Zocor.

Mechanism of action and pharmacology

Simvastatin competitively inhibits HMG-CoA reductase, the enzyme that converts HMG-CoA to mevalonate, the rate-limiting step in cholesterol biosynthesis. This inhibition reduces intracellular cholesterol levels, leading to upregulation of LDL receptors on hepatocytes and increased clearance of LDL from the circulation. Simvastatin also modestly raises HDL cholesterol and lowers triglycerides. It is a prodrug; the lactone ring is hydrolyzed in the liver to the active β-hydroxy acid. The drug is extensively metabolized by CYP3A4, which underlies many of its drug interactions. Peak plasma concentrations occur within 1.3–2.4 hours, and the elimination half-life is about 2–3 hours, though pharmacodynamic effects persist longer due to enterohepatic recirculation.1

Clinical efficacy and indications

Simvastatin is indicated for the treatment of primary hypercholesterolemia, mixed dyslipidemia, and homozygous familial hypercholesterolemia, and for the prevention of cardiovascular events in patients at high risk. The landmark Scandinavian Simvastatin Survival Study (4S) demonstrated a 42% reduction in coronary mortality and a 30% reduction in all-cause mortality in patients with coronary heart disease.2 The Heart Protection Study (HPS) extended these findings to a broader population, showing significant reductions in major vascular events among high-risk individuals, including those with diabetes or peripheral arterial disease.3 Simvastatin is also used in combination with ezetimibe in fixed-dose products for additional LDL lowering.

Adverse effects and drug interactions

The most significant adverse effects of simvastatin are myopathy and rhabdomyolysis, which are dose-dependent and more common with high doses (80 mg) and with concomitant use of CYP3A4 inhibitors such as clarithromycin, itraconazole, and grapefruit juice. The FDA has restricted the 80-mg dose due to increased myopathy risk. Other side effects include elevated liver enzymes, gastrointestinal disturbances, and a small increased risk of new-onset diabetes. Simvastatin is contraindicated in pregnancy and in patients with active liver disease. It interacts with fibrates, niacin, and certain calcium channel blockers, necessitating dose adjustments.

Lesser-known aspects

Simvastatin was derived from a fermentation product of Aspergillus terreus, and its development was led by researchers at Merck, with Alfred Alberts and others playing key roles. It is one of the few statins that is a prodrug, unlike atorvastatin and rosuvastatin which are active as administered. Simvastatin has been studied for potential benefits beyond cholesterol, including anti-inflammatory and immunomodulatory effects, and has shown promise in reducing the risk of venous thromboembolism. It also has a unique interaction with grapefruit juice, which can increase plasma levels several-fold. In veterinary medicine, simvastatin is occasionally used off-label in dogs and cats for hyperlipidemia, though evidence is limited.4
