Parvovirus

Parvoviruses are a family of small, non-enveloped DNA viruses that infect a wide range of animals, including humans. They are among the smallest known viruses, with icosahedral capsids about 18–26 nm in diameter, and their name derives from the Latin parvus, meaning "small." The family Parvoviridae is divided into subfamilies that infect vertebrates (Parvovirinae) and invertebrates (Densovirinae). In humans, the best-known member is parvovirus B19, which causes fifth disease in children and can lead to complications in pregnancy and in individuals with hemolytic disorders.

Structure and genome

Parvoviruses are characterized by a tiny, non-enveloped icosahedral capsid composed of two or three structural proteins (VP1, VP2, and sometimes VP3). The capsid encloses a single-stranded DNA genome of about 5 kilobases, with palindromic terminal sequences that form hairpin structures essential for replication. The genome contains two open reading frames: one encoding the non-structural proteins (NS1, NS2) involved in replication and gene expression, and the other encoding the capsid proteins. Because the genome is single-stranded, parvoviruses rely on host cell DNA polymerases during the S phase of the cell cycle, making them dependent on actively dividing cells for replication.

Replication and pathogenesis

Parvoviruses replicate in the nucleus of infected cells, exploiting the host's replication machinery. The hairpin structures at the genome ends serve as primers for complementary strand synthesis, a mechanism unique among DNA viruses. Pathogenesis varies by species: in humans, parvovirus B19 targets erythroid progenitor cells in the bone marrow, causing a temporary halt in red blood cell production. This leads to aplastic crisis in patients with sickle cell disease or other chronic hemolytic anemias, and can cause hydrops fetalis if acquired during pregnancy. In animals, canine parvovirus type 2 (CPV-2) causes severe gastroenteritis and myocarditis in puppies, while feline panleukopenia virus (FPV) is a related pathogen.

Clinical manifestations and diagnosis

In immunocompetent children, parvovirus B19 infection typically presents as erythema infectiosum, or fifth disease, characterized by a "slapped cheek" rash and a lacy rash on the trunk and limbs. Adults may develop arthralgia and arthritis, particularly in women. Diagnosis is usually clinical, but can be confirmed by detection of B19-specific IgM antibodies or by PCR for viral DNA in serum. In immunocompromised patients, persistent infection can lead to chronic anemia, requiring intravenous immunoglobulin therapy. No specific antiviral treatment exists; management is supportive, and vaccination is not yet available for humans, although vaccines for canine parvovirus are widely used.

Lesser-known aspects

Beyond the well-known pathogens, parvoviruses include several lesser-known members with unique features. Adeno-associated viruses (AAVs), which depend on helper viruses such as adenoviruses for replication, are being developed as gene therapy vectors due to their ability to integrate into the host genome at a specific site on chromosome 19. The genus Dependoparvovirus includes AAVs, which are non-pathogenic in humans. In insects, densoviruses cause lethal infections in pests like the silkworm and are being explored as biological control agents. Additionally, parvovirus B19 was discovered in 1975 by Australian virologist Yvonne Cossart while screening blood samples for hepatitis B, and it was not linked to human disease until 1981. The virus's name "B19" comes from the code of the blood sample in which it was first identified.
