Oncology is the medical specialty focused on cancer prevention, diagnosis, treatment, and research. It brings together laboratory science, pathology, imaging, surgery, drug therapy, radiation, rehabilitation, and palliative care to manage malignant disease across the life course. Oncologists commonly specialize by treatment method, cancer site, or patient group, while multidisciplinary teams coordinate decisions around tumor biology, stage, symptoms, personal preferences, and goals of care.1
Oncology covers the prevention, detection, classification, treatment, and long-term management of cancer. Its clinical work begins with estimating risk and continues through diagnosis, staging, treatment selection, surveillance, survivorship, and end-of-life care. Medical oncology treats cancer with systemic therapies; surgical oncology removes tumors or obtains diagnostic tissue; and radiation oncology uses ionizing radiation to control disease. Pediatric oncology addresses cancers in children, whose tumors, treatment tolerances, and developmental needs differ from those of adults.
The specialty grew from surgery and pathology into a team-based field as radiotherapy, chemotherapy, molecular biology, and supportive medicine developed. Modern oncology also includes cancer epidemiology, prevention science, genetics, rehabilitation, psycho-oncology, and health-services research. The World Health Organization emphasizes that cancer control spans prevention, early detection, diagnosis, treatment, and palliative care rather than treatment alone.1
Accurate diagnosis requires identifying both the cancer type and the extent of disease. Pathologists examine cells and tissue, while imaging and laboratory tests help establish the primary site, spread, and response to therapy. Staging describes how far a cancer has progressed; grading describes how abnormal or aggressive its cells appear. Increasingly, molecular and biomarker testing identifies alterations that may predict response to targeted drugs or immunotherapy, although a biomarker result is only one part of a clinical decision.
Treatment may combine local methods—surgery and radiotherapy—with systemic methods such as chemotherapy, endocrine therapy, targeted therapy, cellular therapy, or immunotherapy. Plans depend on tumor biology, stage, comorbidities, expected benefit, toxicity, and patient priorities. Clinical trials test new interventions and can provide access to investigational treatments while generating evidence for future care.23
Prevention and early detection can reduce cancer mortality, but they do not eliminate the need for treatment services. Tobacco control, vaccination against oncogenic infections such as human papillomavirus and hepatitis B, protection from ultraviolet exposure, healthy living environments, and evidence-based screening all contribute to cancer control.4 Screening is intended for people without symptoms and is recommended only when evidence shows that its benefits outweigh harms such as false-positive results, overdiagnosis, and unnecessary procedures.
After treatment, survivorship care may address recurrence surveillance, late effects, fertility, cognition, employment, sexuality, and emotional health. Palliative care can begin at any stage of serious cancer and focuses on symptoms, communication, and quality of life; it is compatible with anticancer treatment. Hospice generally emphasizes comfort when disease-directed treatment is no longer the principal goal.
Oncology includes several specialized problems that are easy to overlook. Oncologic emergencies such as spinal-cord compression, febrile neutropenia, tumor lysis syndrome, and superior vena cava obstruction can require immediate treatment. Some cancers are defined by a molecular alteration rather than a body site, enabling a treatment to be studied across multiple tumor types; this is called a tissue-agnostic approach. Conversely, cancers with the same organ of origin may differ greatly in biology and therapeutic response.
Long-term outcomes also depend on access, geography, cost, trial availability, and the representation of diverse populations in research. Oncology therefore includes implementation science and health equity alongside laboratory discovery. The U.S. Food and Drug Administration's Oncology Center of Excellence coordinates regulatory work across cancer products and supports patient-focused drug development, illustrating how contemporary oncology extends beyond the clinic.5
