ICD-10, the International Statistical Classification of Diseases and Related Health Problems, 10th Revision, is a medical classification system published by the World Health Organization (WHO) for coding diseases, signs and symptoms, abnormal findings, complaints, social circumstances, and external causes of injury or disease. Developed over a decade of international collaboration, it was endorsed by the Forty-third World Health Assembly in May 1990 and came into use by WHO member states in 1994. ICD-10 replaced the ninth revision (ICD-9) and introduced a alphanumeric coding scheme, allowing for more detailed and flexible classification. It is used worldwide for morbidity and mortality statistics, reimbursement systems, and clinical research, and it has been adapted by many countries into national versions, such as the U.S. Clinical Modification (ICD-10-CM).
ICD-10 uses an alphanumeric coding system with a single letter followed by up to three digits, allowing for over 68,000 codes, a substantial increase from ICD-9's roughly 13,000 codes1. The classification is organized into 22 chapters, each covering a broad category of diseases, such as Chapter I for infectious and parasitic diseases (codes A00–B99) and Chapter IX for diseases of the circulatory system (I00–I99)2. While the WHO maintains the core ICD-10, many countries have developed national modifications to meet local needs. The United States implemented the ICD-10-CM (Clinical Modification) for morbidity coding on October 1, 2015, after years of delays, replacing the ICD-9-CM3. Australia uses ICD-10-AM (Australian Modification), and Germany has ICD-10-GM (German Modification). These adaptations expand the code set to include more detail on laterality, encounter type, and other clinical specifics. The transition to ICD-10-CM was a major undertaking for healthcare providers, requiring extensive training and system updates, and it significantly improved the granularity of billing and epidemiological data.
ICD-10 serves as the international standard for reporting causes of death and disease, enabling comparisons across countries and over time. The WHO uses ICD-10 to compile global health statistics, such as the Global Health Observatory, which tracks mortality and morbidity patterns4. The system's detailed codes allow for tracking emerging diseases; for instance, the code U07.1 was assigned to COVID-19 in 2020 for death certification, facilitating consistent global reporting during the pandemic. ICD-10 also underpins the Global Burden of Disease study, which relies on its categories to estimate disease burden across regions.
Beyond its clinical use, ICD-10 includes chapters for factors influencing health status and contact with health services (Chapter XXI, codes Z00–Z99), which capture social and environmental determinants, such as Z59 for homelessness and Z72 for lifestyle-related problems. The classification also has a chapter for external causes of morbidity and mortality (Chapter XX, V01–Y98), which is crucial for injury surveillance. A notable quirk is that ICD-10 contains codes for unusual conditions, such as V95.4 (spacecraft crash injuring occupant) and W55.29 (other contact with cow), which are often cited as examples of its exhaustive scope. Additionally, the WHO publishes a set of rules and guidelines for coding, including the 'dagger and asterisk' system to flag underlying diseases and manifestations, a feature that aids in complex diagnoses.
ICD-10 remains the most widely used version of the International Classification of Diseases, though the WHO released ICD-11 in 2019, which is gradually being adopted.
