Bioethics is the study of ethical issues in biology and medicine. It examines how values should guide decisions about health care, scientific research, biotechnology, public health, and the relationship between humans and other living beings. Its questions range from consent and confidentiality to genetic intervention, organ allocation, reproductive technologies, and the treatment of animals. Bioethics combines moral philosophy with law, clinical practice, social science, and the perspectives of patients and affected communities.
Bioethics addresses moral questions created by biological knowledge and medical power. Clinical bioethics considers duties between patients and professionals, while research ethics evaluates studies involving human participants. Public-health ethics weighs individual liberty against collective protection, and environmental or global bioethics examines responsibilities across species, generations, and nations.1
A widely used framework identifies autonomy, beneficence, non-maleficence, and justice as important principles, although no single principle automatically resolves every case. Respect for autonomy supports informed and voluntary choice; beneficence concerns promoting welfare; non-maleficence concerns avoiding or minimizing harm; and justice concerns fair distribution of benefits, risks, and opportunities. Deliberation also considers relationships, disability, cultural context, professional duties, and structural inequality.
Human-subject research requires safeguards because scientific value does not by itself justify exposing people to risk. Informed consent should communicate the study's purpose, procedures, foreseeable risks, potential benefits, alternatives, and the participant's freedom to withdraw. Independent review, risk minimization, equitable selection, privacy protections, and additional safeguards for vulnerable groups complement consent.2
The Nuremberg Code, the Declaration of Helsinki, and the Belmont Report shaped modern research ethics in response to abuses and continuing concerns about exploitation.3 Ethics committees and institutional review boards assess protocols, but governance also includes data-protection law, trial registration, compensation rules, community consultation, and oversight after a study begins. Consent is therefore a process, not merely a signature.
Clinical bioethics helps resolve conflicts among patient preferences, medical judgment, family interests, institutional policy, and law. Common cases involve refusal of life-sustaining treatment, surrogate decisions for people lacking capacity, reproductive care, genetic testing, confidentiality, end-of-life decisions, and scarce-resource allocation. Advance directives and supported decision-making can preserve a person's values when direct communication is difficult.
Public-health ethics asks when authorities may limit individual actions to reduce population risks. Measures such as vaccination requirements, isolation, screening, and disease surveillance must be effective, necessary, proportionate, transparent, and fairly administered. Justice is especially significant because health emergencies and unequal access often place heavier burdens on marginalized communities. The World Health Organization describes ethics as relevant not only to clinical care and research but also to health systems and policy.4
Bioethics also covers less visible decisions embedded in technologies and institutions. Questions about artificial intelligence in medicine include explainability, automation bias, responsibility for errors, data ownership, and whether training datasets reproduce social discrimination. Genomic medicine raises concerns about incidental findings, relatives' interests, group privacy, insurance discrimination, and the distinction between treating disease and enhancing traits.
International instruments extend bioethical concern beyond individual clinical encounters. UNESCO's Universal Declaration on Bioethics and Human Rights links human dignity and autonomy with solidarity, social responsibility, cultural diversity, and protection of future generations.5 European treaty law addresses human rights and biomedicine, including consent, genetic testing, and research.6 The field also includes animal research, biobanks, organ donation, disability justice, climate-related health threats, and the ethics of allocating medicines between countries. These areas show why bioethical analysis must include institutions and social conditions as well as individual choices.
Bioethical judgments may differ across legal systems and moral traditions; the principles and instruments cited here provide influential frameworks rather than a single universally binding method.
