Scope and foundations

Audiology addresses disorders of hearing, balance, and related communication functions. Audiologists evaluate the outer, middle, and inner ear, the auditory nerve, and central pathways, while also considering cognition, language, environment, and participation. Hearing loss may be conductive, sensorineural, or mixed, and its effects range from difficulty detecting soft sounds to impaired speech understanding in noise. The field is distinct from otology and otolaryngology, which focus more broadly on medical and surgical diseases of the ear, nose, and throat. The World Health Organization recognizes hearing loss as a major contributor to disability and emphasizes prevention, timely identification, and rehabilitation.1 Audiology therefore includes both clinical diagnosis and practical support for education, work, relationships, and independent living.

Assessment and clinical practice

Clinical assessment matches physiological findings with the person’s reported difficulties and functional needs. Common tests include pure-tone audiometry, speech audiometry, tympanometry, otoacoustic emissions, and auditory brainstem response testing. These methods can distinguish problems in sound transmission from damage affecting cochlear or neural function, and some can be used when a patient cannot provide reliable behavioral responses. Audiologists also assess tinnitus, sound sensitivity, auditory processing concerns, and dizziness or imbalance. Vestibular services may include videonystagmography, video head impulse testing, and positional maneuvers for disorders such as benign paroxysmal positional vertigo. Results are interpreted with medical history and examination rather than in isolation; referral to an otolaryngologist, neurologist, or other clinician is appropriate when findings suggest disease requiring medical investigation.2

Intervention and public health

Intervention aims to improve communication, safety, and participation rather than merely restore an audiogram. Hearing aids, cochlear implants, bone-conduction devices, assistive listening technology, communication training, and counseling are selected according to the person’s goals and type of hearing loss. Cochlear implants can provide access to sound for some people with severe or profound sensorineural loss, but outcomes depend on factors such as age at implantation, auditory experience, rehabilitation, and support. Pediatric audiology links early detection with language, educational, and family services; newborn hearing screening is consequently a public-health measure as well as a clinical test.3 Prevention includes vaccination, treatment of ear disease, safer listening practices, and control of occupational noise exposure. Rehabilitation may also address tinnitus distress and vestibular compensation.1

Lesser-known aspects

Audiology includes several less visible specialties and boundary areas. Industrial audiologists help design hearing-conservation programs, measure workplace noise, and distinguish occupational injury from other causes. Educational audiologists support classroom acoustics, remote microphones, and students whose access to speech is affected even when conventional thresholds appear near normal. Geriatric practice increasingly considers the relationship between hearing, social isolation, cognition, and fall risk, while vestibular audiology may work with physical therapists on gaze stabilization and balance exercises. Tinnitus is not simply an ear-generated sound: its perception can involve attention, emotion, and neural processing, so management often combines education, sound strategies, and psychological approaches. A further edge case is unilateral or asymmetric hearing loss, which can disrupt localization and speech understanding in noise despite normal hearing in one ear. Contemporary frameworks therefore measure real-world participation, not only test-room thresholds.45
